As TMZ first reported, Sutherland struck the woman at a VERY low speed while pulling out of an alley in Santa Monica on June 24. Paramedics raced to the scene ... and treated the woman for minor injuries.

Officials launched an investigation into the incident ... and determined both sides screwed up. 

Authorities believe Sutherland, who told cops he was driving somewhere in the neighborhood of 1 to 2 miles per hour, should have hit the brakes sooner. And the biker is taking blame for riding on the sidewalk ... instead of peddling in the street. A big no-no.

Sutherland collided with a bicyclist in the seaside city, at around 2:45 PM.  The bicyclist suffered some sort of injury -- paramedics wrapped her arm after arriving on scene.

It's unclear who caused the crash.  One thing cops are sure of -- alcohol was not a factor.

KIEFER SUTHERLAND is to be honoured with the 2,377th star on the Hollywood Walk of Fame on Tuesday (09Dec08).The actor’s father Donald Sutherland and moviemaker Joel Schumacher will be on hand to pay tribute to the 24 star at the unveiling ceremony outside the Hollywood Roosevelt Hotel on Hollywood Boulevard.The honour falls four days after
Kiefer Sutherland once tried to take his father's job. The "24" star - whose father is actor Donald Sutherland - says when he realized how much money his dad was making by lending his voice to commercials, he wanted to do it himself.

He said: "My dad does the voiceover for everything from Sunkist to Volvo and one day I thought, 'Hmm that seems a good way to make a living.'"
Donald Sutherland wrote to his son Kiefer "every other day" when the '24' star was in jail.
